本文運用老人周全性評估評估個案身、心、靈、社會之問題，以觀察、會談及翻閱病歷的方式來收集資料，照護期間2012年8月7日至2012年9月3日，確立護理問題為：焦慮及娛樂活動缺失。護理個案初期以傾聽、接納的態度鼓勵說出感受，建立良好護病關係，並應用懷舊理論讓個案回顧以前的生活來肯定自己，依照個案需求安排治療性會談，陪同參與有意義的活動，藉由強化其手部功能優勢，協助找到被遺忘內在資源，重新建立自信及自尊；而經由這次的護理過程，可瞭解到長期照護機構護理人員之多重專業角色對個案的重要性，再者，適當運用懷舊治療，能促進自我照顧能力提升，早日重返家庭及社區生活。

This study used elderly comprehensive assessment to assess the physical, psychological, spiritual, and social problems of the subject, and used observation, interview, and medical record review to collect data. The author cared for the subject from August 7, 2012 to September 3, 2012. The confirmed nursing problems are: anxiety and lack of entertainment activities. In the early period of nursing, the author listened to the subject and encouraged the subject to speak out feelings with an acceptance attitude, in order to develop a good nurse-patient relationship. The author also used reminiscence theory to enable the subject to review past life and have an affirmative attitude toward self. The subject was scheduled for therapeutic interviews according to the subject’s needs. The author accompanied the subject to participate in meaningful activities. The subject was assisted in strengthening advantage over hand functions, finding out forgotten internal resources, and re-building self-confidence and self-esteem. The importance of multiple professional roles of nursing personnel in long-term care facilities can be understood through this nursing process. Moreover, appropriate use of reminiscence therapy can improve self-care ability and help residents return to family and community life early.
